You need the SSBC disk brake conversion from a 2nd gen 1500 with the Chrysler 9.25" rear axle. It will bolt right to the axle housing and you will lose the e-brake in most cases.

Won't work I have tried, 17inch is the smallest you can go before you hit the caliper

Well if you know it takes some modification, shouldn't you know what that modification is?

anything below 17s with not fit on the front. Calipers are too big.

Here's from Matt's thread on the conversion

I bought an SSBC rear disc conversion for the 2nd gen Ram trucks. Its not completely bolt on, but it looks like it will be easy to modify. I'm going to have to drill new holes in the caliper mounting brackets for the speed sensors, have to extend the rear brake lines, and also get rid of the emergency brake.

Why would you want to mount 15 inch wheels on a 4th gen?

This is a drag racing mod; the tallest drag slick/radial you can get is 28" and they're expensive. 15" wheels have a TON of options in both slicks and radials and they're about 1/2 the cost. I can also get a 30"-31" drag slick/radial which better matches my 4.56 gears and I don't run out of rpm like with the 28".
