Transmission slip? (not 1st to 2nd)

What is happening is when really stepping on the pedal to accelerate, (i haven't caught a look at the rpm's as it only happened three times in the last two weeks or so) I hear the rpm's raise and then when shifting it seems to plateau and then shift. I hear it in my exhaust more than anything else as I have a carven competitor and I hear everything. Anyone have any ideas on what it could be or cause this?
