Has it thrown any codes and what does the tac do during the shake? Do any lights come on the dash? Mileage and possible dirty TB. Kinda hard to say without more info. How does the truck also do around town?

What do you consider excessive on fuel? Normal ball park is around 15 MPG on these things. I really doubt the first owner changed the plugs but I could be wrong. They can be a pain to change without the right tools but can still be done. Normal change is 30k or 35k cant remember at the sec but they can go longer if good fuel is ran all the time.

The can but shouldnt be causing alot of your issues, if you do clean it use brake cleaner only and be mindful not to spray alot near the ectertonics. That milage seems low wonder if anything funky is goin on with the injectors.

on the hemi when cleaning the tb should use tb cleaner and spray with eng running or can you clean it with brake clean eng off then start after?

Get some non-chlorinated brake cleaner, works good and wont damage the finish inside the TB. Yes they do get dirty very easy. Check out what mine looked like.

After its clean make sure you calibrate the TPS, turn the key to run, let all the lights go out (the engine light will stay on). Press the accelerator all the way to the floor, let it up and turn the key back off.

The backside of the blade is what gets so dirty like in the pic posted above. Claning with TB cleaner is a 50/50 shot of jacking yours up. The TB cleaner messes up the electronics on these things.

throttle position sensor can also cause this problem had it on a few vehicles, lowered mpg shuddering and surgeing or stalling at a stop and or highway, thats where I would go if it was my truck seen it happen a few times
