Transmission slipping and hard shifting help!!!

I have a 2003 Dodge Ram 1500 QC 5.7l Hemi it has 56,000 miles on it. The transmission was rebuilt at 12k miles and a new one was put in at 21,000 miles. Now it will make a funky chirping noise when put into reverse (and it is not the sound of the brakes) and when it is about to shift from 2nd to 3rd it will slip then shift. When i let off the gas in 2nd it will hard shift into 3rd. All of these problems happen about 1 in 20 times i can go 2 days without a problem then for a day have the problems then be fine the next 3 days. I bought some lucas stop slip (for slippage and hard shifting in automatic vehicles.) I checked the tranny fluid and it is where it is supposed to be. Should i drain 24oz of atf and add the stop slip or just add it to it? Any help would be great ImageUploadedByTapatalk1436828559.844747.jpg
