I agree, Looks like a new radiator is in order. What does the trans fluid look like?

Was the antifreeze orange to begin with? You may want to check the transmission fluid and the motor oil.

coolant with motor oil forms a "milkshake" that looks like a milk shake color

Checked trans fluid, was low, started to fill up and engine overheated spewing out the pepto bismol milkshake. Didn't notice anything in trans fluid other than it being low. Truck has a few small leaks. Engine oil is clean. Radiator was replaced last year, brand new from autozone. Radiator only had water in it when this happened. It's a 01 Ram 5.9 4x4

Distilled. I'm thinking where the trans lines go in the the barrel cracked and fluid leaked in.

You need to replace the radiator, flush your entire engine cooling system that now has ATF inside it, and drain and flush your trans to get the water/antifreeze out.

Perhaps a leak in the trans cooling line that runs through the radiator?
You would be surprised how many people run staight water in vehicles.

by the way my wife had a autozone brand (what ever brand they sell) radiator that wasnt very good lasted maybe 2 years. So i researched and found that most mechanics trust certain brands of radiators. CSF radiator brand makes a good radiator.

Okay, so the engine oil is good? Do an oil change and make sure you don't have any of that milkshake stuff in the pan. Checking the dipstick is NOT good enough.

It is more than likely a bad trans cooler line inside your radiator. Now, everyone who is screaming about killing your trans...take it easy. The trans fluid is at a MUCH higher pressure than the coolant, so if there is a problem, it is doubtful that coolant got into the trans. To be safe, drop the pan on the trans and inspect.

Replace radiator, do a coolant flush and trans flush. You'll be good to go.

No harm in running straight distilled water in a cooling system. Coolant simply allows for more efficient heat transfer.

Running straight water gives you ZERO protection against rust.
