What holds tension on the timing chain on a 1997 5.9 V8? Anything? Is there just no tensioner? That kind of freaks me out, leaves me a bit nervous.

From my experience with my 5.2 which is mostly the same inside, there is no tensioner on the timing chain in is literally just held tight By the distance between the two gears

Don't really need one but you can certainly install one. My 360 never had one and i didn't put one in when i swapped to a double roller. I see it as one more thing to wear out on these
