Define "cuts out". Does the entire truck go dead? or does the engine just die? Sounds like an electrical issue if you can show that you've got air/spark/fuel.

Another possible culprit could be O2 sensors. I'd recommend having someone troubleshoot with a scan tool though...this won't be an easy "driveway diagnosis"
Can you feel a power difference? How is the low end after the header install? Those have 1.5" primaries IIRC which should be really good for lower end torque

The 5.7 is designed to run on 89, so if you consistently feed it 89 it will run most efficiently. I doubt you see any gain/loss on 91/93...it doesn't run aggressively enough in stock trim to take advantage of the additional octane level.
That being said, I average 14 doing mostly stop and go...

There are arguments on both sides...you could make the argument that it helps gas mileage because the exhaust gases displace some of the air in the cylinder which means that less fuel is required to reach a stoichiometric mixture.
Either way, deleting it won't make much difference performance...
Blue smoke is burning oil...coolant would be white. Has it thrown any codes? Start there.
Next thing to check is the spark plug gap. What kind of plugs did you use?
Also - did you replace the head gasket and head bolts when you put the head back on?
If you can find a cummins 2500 for your price range then you're one lucky SOB. However, if you cannot, get one with the 5.7 hemi...those are pretty tough engines as well.
What are you towing? Do you have to have a 2500, or would a 1500 fit your needs?
