Would I fit 37s on a 2500 with just a level and -23 or -40 offset wheels? I'm asking for a buddy. He says his tires stick out about 3" from the fender.

With anything more than a factory offset you're gonna rub by my measurements. Any less than 4.5" backspace will put the tire into the fender. With a -23 to -40 offset I'm guessing you're probly gonna be using a 12" wide wheel? That's gonna make the problems even worse.
It's suprsing how much room you have for 37's with just a level kit.

The tire is 37x13.5x20 I'm trying to convince him to trade my 37s for his 35s

I've seen people fit 37x12.50's using the OEM wheels and a leveling kit. But they will rub on the control arms. As for his wheels, knowing the offset is useless without knowing the wheel's width. But if they stick out 3 inches, he's gonna need a full suspension lift or a sawsall to fit 37's.
