Many flaws are borderline, just good enough to pass development testing only to flunk later in real life.
Companies have done advanced durability tests, they don't catch everything either, especially on very new and different designs.
Statistical probability studies are supposed to be conducted to avoid, but these take time and money.
And we all know who hates that.
