Unfortunately, you do need access to both CAN lines. The reverse switch signal comes in over CAN-C but the messages to display the backup camera are spit out over CAN-IHS.
So thanks to @Roman217 and @ManualCummins issue #2 is resolved! Powering off of an accessory run circuit rather than a full-time hot circuit fixed this.
Since there's no key switched power behind the radio that might be out so the BCM might be the next best thing. I'll fiddle with it some more.

That wouldn't be too hard to add, actually. What's weird is my backup lines disappear after the 10 second delay but when I swap in a 2018 camera the lines stay all the time. This function seems to be part of the camera and not the radio.

That'd be great! I'll message you! Having it in the OBD port is convenient for developing this, but I won't want it there long term. I've already got a CSP4 switch attached to it and I brush that every now and then when pushing in the clutch. Back of the radio should be much cleaner.
I've got issue #1 sorted. I had a typo in the code that was causing it. Fixed it and now you can manually turn off the backup camera, shift out of reverse, then go right back into reverse and it works.

Thanks bud, I've got a premium cluster already but what's this with the song title and TPMS stuff? My TPMS works perfectly with my cluster but certain satellite radio stations don't show up...is that because of the year of the cluster? I got the thing off eBay and am not sure what year it's from.

Confirmed! I can turn my backup camera on and off via the CAN interface. I've got a dual CAN board showing up this week. I'll program it to read in the state of the reverse switch and automatically turn on/off the backup camera.
I'll want to run it in my truck for a while to make sure there's no weirdness with the CAN communication. If it all works, though, I'll design up an enclosure, list parts, and share the code so anyone can tackle this if they want.

So @Roman217 has been helping me with CAN communication. We're able to get rid of the "Camera System Unavailable" message and replaced it with the "Check Entire Surroundings" message so that's something. Still not turning on without using the soft key but making progress.
Alfa doesn't show any changes in the Radio status and there's nothing coming in on 0x170 when I shift into reverse.
