Turned out to be a plenum gasket leak (also referred to as a belly pan gasket, connects the plate and the manifold together), and/or a bad crank position sensor.

I want to replace the crank position sensor on my 1993 D250, 5.2L auto. I have found the wiring pigtail for it but can neither see nor feel with my hand the sensor itself, neither from above or underneath the vehicle. There's no clearance for a tool.

The heaters will not actuate with a hot engine (they're not needed with a hot engine). The amount of time the heaters (two) initially actuate before cranking and then cycle after starting is dependent on the engine temperature.
1993 D250, 5.2L, auto trans., 110K miles. I rarely drive it, it's garage stored and I only use ethanol free rec gas with fuel stabilizer. This truck is rust free and all the underside components (suspension, bushings, isolators, brakes, etc.) have been recently replaced - drives like a brand new...
The A/C compressor in my 1993 D250 developed a hole in the housing and lost all its freon. I've been told that small metal pieces probably got circulated into the condenser, A/C radiator, drier bottle, lines, etc. and simply replacing the compressor is not a viable fix.

A component of battery life is temperature, hotter climates have much lower expected average battery life.
On a dual battery application, the batteries are in parallel via large gauge cables and are effectively/electrically one battery. Differential in battery life may be due to one battery...
Besides the towing/hauling weight requirements listed above, there are max. temperature limits that are dictated by towing/hauling ratings. These include trans. fluid, engine coolant, engine oil, and differential fluid. As the weight ratings increase, cooling systems become more robust.
I'm a retired Ram/Jeep engineer that was in charge of the battery/starter/alternator system. If you need more than 400W you go directly to the battery terminals, or on later models the "HAPP" (Hi Amperage Power Point) if so equipped - and then use an aftermarket inverter.

The air pump being part of the emissions system, an OEM will never explain how to remove/eliminate those components. That could land them in hot water with the EPA.
