What is obvious to me is not always obvious.
Tumbleweed pointed out 6 other things :)
A professional does NOT ASSUME!!
Thanks for making sure I identify the proper part.

Thanks, Correct me if I am wrong here but did that hose originally consist of metal piping as shown in the diagram below and feeds the heater core?

I have never seen a Cummins with the set up in the OPs Picture.
Anything is possible but I don't think it is OE. Likely someone replaced the factory setup with the one in his pic.

I'm not sure Benny. we just got the truck, this hose feels like it extends to the back of the engine, I'll get some more photos/perhaps a short video tonight. The heater core appears to be off a different line.

This certainly is throwing me for a loop. I've been researching 5.9l diesel photos and cannot find any hoses that run from the thermostat area to the back of the head. Furthermore in looking at photos of the cylinder head I do not see any water port directly below the thermostat housing, Im starting to think this was modified and tapped to allow for more cooling perhaps?

Im certainly no Diesel expert but it would be great if someone could chime in here to help. I will keep researching in the meantime.

No problem. It doesn't look like the best design though. Woven cloth covered hose right above the exhaust manifold with no heat shield. That sucker probably gets 800 degrees and heat rises cooking that hose especially at idle. Steel braid might help but there is still rubber hose inside the steel braid.
