Your cabin air filter ?
I am usually once a year. BUT all depends on where you live. If we have a bad pollen season I may do twice a year ! But I pull the CAF about every 6 months to check ! I usually have a spare in my truck as I buy 2 at a time

are they dirty when you change them?
IMO The change time varies on where you live !
If you wake up every morning with your car covered with a heavy coat pollen like my friends in Daytona change it more often
I would check it every three months to start then once or twice a year and see how yours looks and set up your own time frame.
If you like gadgets you can also pick up a Anemometer and check air flow ! IMO not 100% perfect way to check but can give you a idea !

Once a month I do all my checks, fluids & filters. With the German Shedders and the dirt roads, I usually take a vac to it every other month and change at oil change (10k). Next time will be getting the K&N

Most of the time not too bad but its worth the peace of mind for the relatively small amount of money that it costs. I have some pretty bad sinus problems not to mention the pollen down here in the Spring is pretty bad too. Besides I've had my truck for 3.5 years and have 33K miles on it so I've changed both filters twice.

I like RADO's answer. Once a year or so. Have a spare on hand, ( I buy the charcole activated ones Arm and Hammer Fabreeze) When you check it, if it's discolored with dirt, just swap in the spare. It's cheap insurance that your not clogging up your heater core or ac evap core.

Once a year now that it gets used year round. Not really very dirty even then.
