Knocking?

1999 5.2l, 115k miles.
Recently started making what sounds like an engine knock at idle, but if you increase the rpms or put it in gear (automatic) it goes away. Anyone have any experience with this?
I was thinking maybe an exhaust leak, especially since the same time this started the muffler self destructed at the seams (new exhaust on order) but the fact it completely stops when you put it in gear or rev it a little bit has me lost. But when you crawl under the truck it really sounds like something in the bottom end, but i know exhaust leaks can make some crazy sounds some times.
Just curious if anyone experience anything like this and could pinpoint a possible cause to look at.

Without hearing it, my guess is lifter tick. That can go away when you rev, but exhaust leak doesn't go away with revs, if exhaust leak goes away it depends on heat of the engine.

It's like we get a new case on this forum every (other) week
