Only 1 of those tabs was broken the first time it fell out. I had never removed it before the first time it fell out.

Anyone have issues where it revs up real high 3-4K rpms when starting from a stop? I turned off hill assist thinking it was related, but it still happens sometimes. If it has low gears beyond drive, I would have guessed I put it in one of those gears.
