On the 2020 Ram 2500, what’s the difference between the stock 18” tires and optional 20” when it comes to ride quality? Don’t plan to do much off road, mostly towing a travel trailer. Thanks.

Much smoother is a stretch, I think I've had both and there isn't enough difference to eliminate 20's. Your carrying more psi in the 3/4 and 1 tons which makes both stiff sidewalls. Get the look you like.18” will be a much smoother ride.

Tires on my truck are 275/60/20 and my spare is a 17" What the heck...! Looking for 17" or 18" rims and tires

I bet if you measured that 17" tire, it would be the same overall size (rim+rubber) as the 20", or very close. My father had the same thing on his 2015 Laramie. My 2014 Longhorn, before I traded it, came with a 20" spare, but only because it was added as an option back then.
My new 2020 came with 22" rims and an 18" spare.
