Whats' the Difference in Fuel Economy Between a Cummins 6.7L H.O. and Standard Output?

I get approx. 14.7 on slightly hilly terrain, mostly in town. The best I have gotten was 15.9 on a trip with a car trailer to WA and back to AZ. I have 3.42 rearend with an RE66 Trans. My 98 2wd Ram would consistently get 22 mpg I don't what gears it had.

Same. If all the variables were identical, including the amount of work the two engines are asked to do in the same time frame, they would use the same amount of fuel.

What exactly, MC, is this "dealer tune" you speak of? You attribute your great mpg to it? I'm interested.
Goose, perhaps calling it a "dealer tune" could be a bit confusing to some readers. I was mostly trying to clarify that I have not yet upgraded to a custom tune designed to add horsepower. (Although I am still considering it to reduce some of the pedal lag, etc)
I wrote a bit about the issue on https://hdrams.com/forum/index.php?threads/annoying-jerking-issue-update.1265/page-5. For those that can't access that forum link, from the factory my truck had a horrible surging problem like many others. I'll paste the basics below:
Problem: "surge" every 0.75 to 1.5 seconds if under a bit of load. Regardless of whether on cruise control. Surge would cause turbo boost to jump 1-4 psi each cycle. Regardless of altitude between 3500 - 7000 ft, my symptoms were unchanged.
Vehicle: 2020 Ram 3500, purchased Dec 2020 with HO & AISIN
Dealers: the truck displayed this problem from Day 1. Two dealers told me to just wait for the computer to eventually learn my driving habits. (wrong). After about 10,000 miles, I finally read all the ideas on this forum about the injector connections, the TSB for the CP4 pumps, and checking tranny fluid levels.
Solution: Took it to a 3rd dealer and requested the TSB 18-059-20 flash. Their report says they instead performed TSB 18-012-21 Rev A on June 7 2021. "Updated PCM software 524010032AG-AK". I'm interpreting that is a full software update. Seems to have resolved 95% of the surging problem. The only remaining problem is without a trailer, but with a modest incline (say a 2% grade) at highway speeds. At that situation, most passengers will comment "why is the truck kindof jumping?" But if instead I had a loaded trailer, or are intentionally accelerating, it smooths out and is nearly imperceptible. I have not noticed any obvious degradation in mileage or power after that flash. So, Goose, I am NOT trying to say that the new PCM software improved my fuel economy. It was fantastic from day 1. I'm saying that the PCM upgrade version I received didn't crater it like some other owners reported when trying to address the surging problem.

As I was posting in this thread, I'm still pleased with the mileage. I remain tempted to try to eliminate that last 5% of the surging, but fear it could drop my fuel economy down to what others are observing.

John, I see you have the HO w/ a 3.73 Axle. What is the best highway mpg you have gotten, empty?

I take a two week trip every year from the San Diego area to eastern Oregon, all freeway and highway. In a 1895 mile round trip, I average 15.7 & 16.0. There are 4 fuel stops; 326 miles @ 15.5, 623 miles @ 15.5, 522 miles @ 16.6, 424 miles @ 15.3.
Approx 1550 miles @ 70-80 mph and 350 miles @ 65-70 mph.

I only have 9747 miles, maybe still breaking in. This year I changed from Falken Wildpeak A/T3 tires to Continental TerrainContact H/T highway tires. And I've converted from carbon/fossil diesel to Renewable diesel. The trip is coming up in September, anxious to see if I've made a difference.

That's about what I have gotten, empty, in mine. Avg 70 mph. But I have 4.10 gears.

Did the fuel conversion require anything?

Here is what I was getting.

2012 Ram 2500 (6.7 HO, 68rfe, 4wd, 3.73 gearing):

Pre-Delete/Tune:
Empty - 14mpg
Towing 11k lbs - 9-10mpg

Post-Delete/Tune (stock HP tune):
Empty - 19 mpg (have gotten up to 21 mpg on a long interstate trip)
Towing 11k lbs - 12-13 mpg

2016 Ram 3500 (6.7 SO, 68RFE, 4wd, 4.10 gearing):

Pre-Delete/Tune:
Empty - 14.5mpg
Towing 15k lbs - 7-8mpg

Post-Delete/Tune (stock HP tune):
Empty - 18 mpg (got it to 20mpg on a long interstate trip once)
Towing 15k lbs - 10-11 mpg
