5 speed to a 6 speed on a 2500 4x4??????

I was wondering if anyone has any input or experience going from a 5 speed manual to a 6 speed manual in a 2nd gen. 2500, V-10, 4x4???
Will a diesel trans work ? what years? difference in transfer cases or changes needed to install a 6 speed? driveshafts. Just any input would be appreciated.
The reason is my 99 Ram 2500 4x4 has the same problem the 5 speed diesels have. No gear between 3rd & 4th. when towing in the mountains 3rd gear will pull up anything but 4th just lugs you down too much. I'm told by a friend that had a Diesel 2500 4x4 Ram that she had the same issues. When she bought a new truck in 2000 she ordered it with a 6 speed manual & that made a bunch of difference in towing. That basically the 6 speed gave you that gear between 3rd & 4th. or a lower 4th gear possible.
