I have had a bad heater core do this one time. It didn't leak but must have been restricted because it acted the same way you are describing. Replaced it and no problems

I had this same exact issue. Replaced the switch and checked the controls too. I replaced the blower motor itself and haven't had an issue since.

It sounds like it might be a broken valve spring. I had it happen on my 03 and it ran like crap and threw o2 codes I assume because it was running so poorly.
It looks to me like the ring gear is too far away from the pinion gear. You need to loosen the passenger side adjuster and tighten the driver side up in small increments.
