What exactly is the difference between putting a level on top of a lift for a combined 6" versus getting a 6" lift

You can't put a level on top of a lift. I'm not sure what you can do regarding bilsteins and a lift. It's only like 200 to go from a 4" to a 6". It's not only cheaper but easier to just buy the 6 inch.

Now if you were trying to figure out the cheapest way to do this..well there isn't one. However the more cost effective way would be a 2" level with a 3" body lift for a total of 5" of lift. This will still cost you close to $2000 though. Here's what it looks like with 35's.

I agree when looking at lifts for my 13 4x4 the difference of only $200 for the 6" in all the brands I looked at.

A 6" lift will keep the correct suspension and driveline geometry (diff drop, sway bar links, etc.)
You would also have to do something with the rear if you did the 4" kit too. Most lifts will leave your truck level so you'd be nose high with the leveling kit

You can run a level (strut spacer style or leveling struts) with a lift although pretty much every manufacturer says not too (liability and saftey)
Some kits come with preload spacers that have to be removed
