Broken rocker arm rail.

I just replaced my cam and lifters on my 2014 5.7 hemi. After startup i was awarded with misfire codes. Not sure of where my screw up was i tore it back down and found that i had a broken rocker arm rail, i was sure the push-rods were cupped. I'm not sure if it was in the torque, or what. my question "has this happened to anyone else. and do they know why. any help would be appreciated.

One question is it in time, possible bent push rods or bent valves?

no, no bent rods. i did not check the valves. the new rocker rail will be here today. damn now i am spooked as to going forward. would over torquing be the cause? or maybe a loose bolt?
