How was this one done? This piece doesn't look to be removable?

In the laramies it is removable. Just plastic welded on.

So how does it come off? Cut it out? And how did you reinstall?

Take the door panel off, and on the back of where that piece is there should be circles with dots in the middle. Lightly drill out those dots and it'll come off.
