That wheel arch is I have seen is a design problem and doesn't have a good drain to keep out moisture.If you replace the panel I would some how put some kind of rust preventive on the back to keep from rusting.

Take your tail light out and take one of those bore camera and it will let you see how rusty your truck is.Can you do body work ?If so cut it out and replace the panel.They make a rust neutralizer it will convert rust into black rust and it can be used as a primer.I got a little cancer on my wheel arch I'm going grind it back and fix it so you are not the only one with this problem.Heck I have seen worse on a Chevrolet it aint just ram.Just fix it before it gets to bad.

Fix it now before it gets real bad.
Pull the tail light and flick the living crap out the wheel well.
You'll nearly have a stroke when enough dirt and sand come out to fill a 5 gallon bucket.
Treat from behind with a rust proofer.
