I would definitely start with cleaning it but that looks more like a bad throttle body...

First things first. I unscrewed the throttle body 8mm bolts by hand (wtf to previous owner). Here is before cleaning.

Test drive = All good for now

Did a pull from stop and went 1-3 full band and no CEL. Misfire on #8 but injectors with almost 220k might be the issue there :)

I'll drive it to work tomorrow and report if I had to pull over and pull the TB plug or not :)

Sounds like you figured it out, needed just a lil cleaning. The fact the TB was on finger tight may have definitely caused some issues. Now that you might have to replace an injector, best to do em all, might even upgrade them to a higher CC flow (for future upgrades)

Perhaps, but remember that's a plastic intake manifold. Too tight and you'll strip them. From the before pic it does look like the OP had some leakage however.

I have the same codes ..cleaned the TB and no luck, changed the TB and no luck...I can reset it by turning key off and the ETC light goes away but reappears as I drive. I disconnect the battery, reconnect and drive about a mile or so and it reappears...SMH I have a 2012 5.7 hemi. Any ideas? any help would be appreciated

While I agree that you shouldn't install the throttle body with a 10' cheater bar, I think it's suppose to be a little tighter than finger tight. One thing you might consider for the future is a catch can. It sure will help keep your throttle body a little(lot)cleaner.
