Cancel both Mopar pre paid maintenance and Century extended warranty

Does anyone have the forms that need to be filled out to cancel both the Extended Century warranty and the Mopar pre paid maintenance plans? We emailed and called our dealer where we bought the truck and the guy who financed/closed our deal said he would email them to us but we haven’t gotten them. I bought the truck just a few days ago. Idk why I agreed to them, we HAVE ALWAYS turned such things down..
Any advice on how to cancel is welcome. We live in California so we can legally get 100% refund if cancelled within 60 days.

That's what I did after my 2014 Big Horn 6.4L CC was totaled. The office manager even helped me with the process. In the case of my extended warranty provider they required that the selling dealer file the paper. I have no idea why but I was glad for the help as I was busy shopping for a new truck.

In the warranty paperwork it states that you can go to ANY Dodge dealership and they will provide or file the paperwork for you. It does not need to be the selling dealership.
