Newbie looking for some guidance

I just picked up a 02 Ram with 5.9 quad cab SLT with an automatic tranny.
I love the truck so far and have stopped looking at the MPG because it is not my daily driver,it is completely stock and the body and interior are in good shape.
My questions are related to the unknown for me...the previous owner had a 5th wheel hitch on it. So far I have not noticed anything abnormal for these models aside from what I read, the shudder during acceleration and a little rough shifting. Should be concerned about the tranny?
My other concern is during idling... are these rigs known for any type of exhaust manifold leaks etc? The noise I hear from the engine bay is consistent with what I hear at the tail pipe, my buddy likes to fear the worst and thinks it is an issue with the low end of the engine.What do you think.

Could be a cracked manifold, doubt the bottom end is hurt unless the previous owner never checked or changed the oil. Welcome to RF.

5.9 has a very strong bottom end. prolly not the problem.

the 46re is not the strongest trans. you could have trans issues. post a video, lets hear it.

you can check for some broken manifold bolts. that could cause your exhaust leak sound.
