When you do a gear swap from 3.55 to 4.56 do you need to retune for the 4.56 if you have one if the canned tunes installed?

No need for tuner adjustments after a gear swap, only tire size change.

(speed sensor is in the ABS tone ring where 1 revolution still equals 1 revolution regardless of ring/pinion ratio)

When changing tires sizes with the tuner, I'm guessing you have to measure the true diameter of the tires with the vehicle weight on them first?

Not really, best bet is to enter the metric tire sizes,
For me it's 305 for width, 50 series, then wheel diameter 20".

Then to confirm test drive it and check against a hand held GPS or highway mile markers for accuracy.

Diameter doesn't really factor in, mounted with weight on it or sitting in the air the circumference shouldn't change.
its the distance traveled per rotation that matters (aka revolutions per mile)
