Not sure about miles, but the best way to inspect the belt is by looking at the back side for deep cracking. Other than new, a used serpentine belt will have a little cracking, but when the are many cracks and they are deep into the belt material it should be replaced.

Currently I have 194,000+ miles on the OEM serpentine belt, and it still looks good. (12 Ram 2500 with the 6.7 Cummins)

WOW guy, lots of miles on a belt. I would carry a spare in the ride. But that is just me, and I's a BOIRD.

I just bought a new Gates Fleetrunner Heavy duty belt just to have a replacement if needed. The owners manual says to replace at 100,000 miles. If the original lasts that long that will be great.
I would just hate to be stranded out in the sticks just because I didn't have a spare belt.

YUP, and depending where U R, it may be interesting to find a belt. Not like the old days with different fan bets and options. Even though all the new bling is great, and electronics and such, sometimes I do miss the simpler times.

I have 122k miles on my truck and it's 6 years old. I just replaced my belt, idler and tensioner two weeks ago. The belt still looked new, the tensioner was good and tight and the idler had no play in it. I was really surprised I thought they would've shown a lot more wear. Hopefully these new parts will last through 250k miles.
