Thunk and Violent Shaking when Accelerating

The truck had been going "thunk thunk thunk" when aggressively accelerating below 20mph.
This weekend I towed ~20k lbs through some hills and it got much worse. It's now a violent vibration and it occurs at all speeds when accelerating.

I'm guessing it's a driveshaft issue.
Looking down there I see some kind of collar at the base of the cab that looks damaged. Pictures attached.
Could that be my problem? And if so, what's the part number?

yes that is in fact your problem you need to change the center support bearing on the driveshaft I would do new ujoints while its apart.
Yep - Do it once, do it right or you'll just be in there again before long.
