What Can Make O2 Sensor Go Bad

I had a P2098 come up in September. After some work diagnosing things, the live data from my code reader made is pretty clean that the Bank 2 Sensor 2 was not giving correct readings (sticking in the .03 to .09 range) where from what I could determine it should be much higher like .45ish. I replaced the sensor with a Denso since it is a brand I have had success with in the past on other vehicles. That fixed the problem. The CEL went away, and the reading in the live data where well within specs not to mention my fuel economy improved.

Now, beginning of November I was out and my CEL went on. I checked the code, and sure enough P2098. I checked the live data and it was back to what it was with the other sensor.

Other than perhaps having gotten a factory defect which I have had with other things I have purchased over the last 20 years while fixing my own vehicle, what are possible things I need to look into that might has destroyed this O2 sensor. Note, the upstream one is working perfectly as are both of the Bank #1 O2 sensors.

I had a bad O2 sensor on my 03 2500 5.7. replaced O2 sensor and CEL came back on a month or so later Turned out to be a bad wire, had to replace that.
