i have no idea what this is, but it was open. It can also close. So I’m wondering what it is and if it should be open or closed.

I figured you were talking Cummins since your picture says 07 & 5.9
PTO = Power Take Off
For powering hydraulic pumps, water pumps, auxiliary stuff etc
