My 5.7 runs rough when accelerating but smooths out when coasting and a scan tool shows “multiple cylinder misfire”. Would this be internal on the engine or possibly an ecm issue? It does it warm or cold but of course our intake air is a bit chilly this time of year.

Whens the last time it had the plugs changed.Start with the basics, make sure all the coils are plugged in properly and it has at least a decent set of spark plugs.

Maybe the spark plugs or coil packs are bad. 16 spark plugs and 8 duel coil packs would be the first place I would check.

Okay, plugs replaced(definitely needed) and coils moved to other positions and I still get a miss/hard vibration at medium acceleration (“multiple cylinder misfire”). If I floor it or let off the vibration goes away. I have heard something about MDS actuators that may not be working right. Is this a possibility or is it maybe a computer/ecm problem?

Could be a cam or lifter problem too

I cringe every time I see one of these misfire threads. Went through this on my old 07, threw parts at it for awhile and finally took it to a mechanic. $2500 later and it turned out to be cracked injectors. Lets just say I replaced everything but the cats. I highly recommend you have a mechanic look at it if you don't have all the tools and time to diagnose it. You could get lucky swapping a few of the basics, but it is a gamble.
