2004 1500 5.7 accelerating tick?

Recently inherited a 2004 Ram 1500. Strong tick coming from the passenger side engine compartment. I’m about 99% sure it’s not a traditional “hemi tick”. It only happens when in gear and accelerating. Cannot be reproduced in park or neutral making it very difficult for me to track down. Any ideas?

I had same sound under same conditions in my 2003 5.7. turned out to be three broken header bolts, they are notorious for breaking. My mechanic had truck 2x before finding first one and while replacing found 2 more he couldn't see at first.

as mentioned above id put money on exhaust manifolds 3rd gen hemis may make some minor idle noise but they dont have lifter issues.

chances are you can feel the flex in the heatshield if the header bolts are broken

and it does seem like the passenger side goes first

good excuse to put full length headers on
