Well, this just in from the dealership... Most likely it's the blower motor and whatever else controls it all down under there. Which means the entire dash has to come out. Long story short, $1400... Now to see if I wanna attempt this myself (YouTube is great) or find a less expensive shop to do it.

Or, the wife says "what if we trade it in for a new truck?"

hey just took the time to read your thread and yes it seems that we have the same problem... now like you said you ruled out almost everything on the hvac at least from the interior blend doors changed and control head changed... now you said you took it to the dealer and thats what they told you? blower motor? if thats the problem the only two thing that I found on this truck related to that are the resistor control module and the blower motor itself both of them are accesible from beneath the dash If I remember correctly... but 1400? from what? I dont see the blower motor costing more than 100 new nevermind the resistor module which I have a pic on my thread by the way

Ok, update on this situation. Finally got it fixed, but not by the dealer. Came across a former service tech specializing in automotive a/c. Anyways, after taking the seats out, moving the center console, and removing the dash unit, he was able to get to everything.

The culprit was the passenger blend door motor. Nothing wrong with the blower, blower motor or anything there. Just the passenger blend door. He replaced that, tested everything else out, and it all works perfect. Not a single hiccup in the system, period! Here's a few pics he took while performing the work...

What did he charge you if I may ask? That looks painful but not impossible. A lot of time I am sure.

More than I was hoping to pay, but he charged me $1000... But he also recharged system, coolant, other thing too, but most of it all was labor. I gave him the truck Friday night. He basically spent all day Saturday and Sunday working on it.
