Drive shaft play and noise

I've recently noticed a squeaking when backing up, and a bearing-grinding noise at above 50 mph. I checked the u-joints, and found that the front of the drive shaft has some play where it goes into the tail housing on the transmission. I cannot find anything on-line about a bearing or bushing inside of the tailshaft-housing. Is there something in there that I can replace, or do I need to replace the housing? It's in an '08, 2wd quad cab, 5-45RFE trans. There is also a wobble at around 65mph, which I thought was alignment, but didn't go away after having it re-aligned. I'm guessing this may be the driveshaft, also.

Update:
I removed the extension housing, and there are two bearings and a bushing inside. There is a very large bearing towards the front and a smaller one in the middle. The larger bearing in the front definitely feels worn, not a lot of play in it, but spins a little roughly. I was able to get a new seal and bushing from Autozone, however, no one seems to have the bearings.

Just pull the bearings out and take them to a dedicated bearing store.They'll just measure them and give you a replacement Unless there's something odd about them like a snapring groove.
