Door latch cable

Is there a repair kit available to repair the cable end that attaches to the lever?
Apparently there are different replacement cables also based on the door size.
Is there a difference in the drivers doors between a crew cab and an Express cab?

The CrewCab and QuadCab front doors are the same but the Regular cab door is longer.
Express is a trim level, the door panels are different by style but are same size. You can put Limited trim level door panels on an Express of the same cab size.
