Water would accumulate behind the rear seat and travel down the left rear and left front door sill which would then accumulate under the driver side floor matt. The cause was a rear window water drain hole that plugged up with dirt which caused the water to drip down inside the cab. To fix this locate the drain holes , there are two - left and right 4mm x 10mm. They can be seen from the outside of the cab and are located below the rear window glass about 8 inches from the outboard side. You will see a small rectangular hole in what appears to be the lower horizontal glass rubber seal. This hole allows water to drain from the sliding window guide track. You can check to see if water is draining by using a small syringe filled with water and while inside the back seat squirt water into the track, the water should exit out the small rectangular hole mentioned above. To fix use compressed air directed at the rectangle hole.
It took over a year to find this simple fix and another six months to get rid of the mildew smell. An Ozone generator eliminated about 90% of the mildew smell.
