Appreciate the help fellas! My next step was to in fact take out one of the covers on the dash to check the part #. I guess the joke is on me here...

Taking this piece out of the dash finally let me figure out how to get it to sit level when installed. It's been crooked since I bought it. If you stick your finger under this tab, it stretches it out and lets it sit flush with the dash.

This is funny.. makes me want to go test drive random vehicles with a pocket full of bolts, that I will hide under the seats or other random places, for the future buyer.

Lol, the only thing I can figure is it must've been underneath the carpet or trim somewhere (had it all apart to install heated seat kit). I've never seen it when I've vacuumed before and there aren't too many places to hide in a regular cab.

@SitKneelBend, did you figure out how to get it to sit flush. I went around online and say someone mention an advisory on these things from the manufacturer. I couldn't find anything though.

What did you end up doing?
