Installed the Bilstein 5100's at 2.8" and added a rear 1.5" spacer. Alignment guy says the rear is off center. Do i need to install an adjustable track bar? That way i can bring the rear back to center.

Measure yourself with a tape and see how much difference you have side to side. You shouldn't have much with just 1.5" rear lift.

I change my 1.5” spacers to 2.5” Shifted over 1/4 3/8 more to passengers side if I had to guess he’s probably about 1/2

Is it off enough that you can see it, or just that you can find it if taking time to measure? I have a 1.5” spacer in the rear and while I know it has to have thrown it off a little, it’s not enough to notice so I’m not worried about it. A small variance of 1/4 to 1/2” won’t hurt anything, so it’s only worth addressing if it bothers you. I don’t usually mess with track bars until I’m getting into lifts of 3” or higher, which is when it gets really noticeable.

My 14 was off when untouched so I bought an adjustable track bar just because I am a bit **** about some things. It was bad enough the paint on the bed didn't match the cab .

My 1.5" lift in the rear shifted .6". I couldn't tell by eye; but once I measured it, it could see it every time I looked at the back of the truck, lol. I had to readjust the the Core track bar and get it centered.
