Who touched the wheel last? Are the others torqued properly? Maybe someone needed a lug nut and stoled it?

yeah, I'm fairly certain it's been gone for about 3 months, that's the last time I had the tires rotated. So, they either lost the nut, forgot to put it on, or stripped it out and just didn't bother to tell me.
