Tow hook install - long bolts won’t drop down

I purchased tow hooks. Got the old cutouts removed and new bezels in. But, when I take out the 10+“ long bolt with the 18mm head, they unthread about 2“ and drop down about an inch and appear free but won’t drop down so I can remove them. Any idea what holds them in at the top?

There's a retaining washer holding them in place. When I removed mine to paint them, I put vice grip pliers on the bolt, then tapped the pliers downward with a hammer to get them out. It distorts the washer enough for the bolt to fall out. I think there's some youtube videos out there showing this.
