I would think that as long as it was serviced on time and properly maintained, 54k on a 6.4 would be fine. I haven't owned one though, so take it with a grain of salt.
I wouldn't be concerned with a 7 year old truck that was seeing 18k miles a year. My prior 3500 was 3 years old when I bought it and it had 98k miles on the ticker.
Dedicated gooseneck hauler, but pristine condition. To this day I still regret ever selling that truck. I WOULD ask them why the CEL is on though.... I would also ask if the 8.4 UConnect screen has been replaced. 2018 is the year that the radios had problems with screen delamination. Neither would sway my decision if the CEL issue is resolved and the truck fit my needs.
127k is what the truck in my sig has now. Engine wise, that is almost perfectly broken in IMO. However, emissions should be a concern around that mileage. If you use it primarily for towing, then you may be ok for a while longer but if it's a daily driver with occasional tow use you can expect it to hit the fan sooner than later. There are cost effective methods to remedy that, but it depends on your comfort level. I bought this truck out of Woodstock, GA. That dealer primarily sells diesels and they had a few that had already been "fixed" if you're looking for that. Totally up to you and I won't steer you one way or another.
