So I ordered the Bilstein's for the front of my lifted Ram. How do I measure to see how high I need to go?

Measure from the center of your wheel to the lip of the wheel well in the front and back and the difference will tell you how much rake you have

Looks like just a hair over 1.5 inches of rake. The truck has 21k miles on it, and it's been lifted for about 19k of that. Do I set it right at 1.5 or do I go a little higher to account for any sag?

I know this is a bit late, but they only have a few settings (0.7, 1.4, 2.1, 2.8) so I would say go for 1.4"

I had 2" of rake and set mine at 1.4. It has a little rake still but I can load my tools in the box or tow my trailer and not be nose high. Something to think about if you tow. Looking at a larger trailer so may end up with airbags anyways, they're cheap so no biggie
