Height wise they are pretty even, there's some overlap side to side, but it's only noticable on objects that are right in front of you. But, there's also a left/right adjustment that. May take that out, but I haven't felt the need to do so.
If you can post a pic against a wall, i just wanna make sure i didnt get a bum set lol
Mine are definitely different heights, outer led above inners and you can see it in this pic, so the inner leds bring that bluish top cutoff into my outers white cutoff. Easy to see the difference if you cover one led with your hand.
Its not terrible and the lights are bright, but the high and lows should be on same plane to me, if others are the same then i want mine that way also!

Everything looks pretty good based on what I can see there. Yours actually looks a bit better than mine.

Ok, could be just the pic why you think mine look better lol iphones tend to be a little more warm.
These are vor, i adjusted them to vol which is the left cutoff. Need to adjust again to the right side of cutoff. Id have to imagine they are tooled all the same and each led projector has its own spot it clicks into place, maybe they are designed to be off plane a little bit, who knows! Im sure these are brighter then any single led housing other than customs. I love the look and that the black is gloss to match the other gloss black around the truck.

I think my housings were a set of pre-production test pieces that were sent out to select dealers to get feedback (which I why I got them cheaper than what they retail for). So it's possible the projectors aren't aligned as well as actual production units. Here's mine with the lows and highs on. You can see that the cutoffs for the individual driver side (and also passenger side) projector aren't 100% aligned. I might be able to tweak it, but the left/right adjustment has to be done with the housing off the truck. Splitting hairs, but they are still leaps and bounds better than any Alpharex housing I've used.

Lowered mine again to right cutoff @39”
Better pick of the height differences between the 2 leds in same housing.
Not sure why it is this way when there is no difference in height on the truck, left to right they should be off but to me the cutoff heights should align between the same housing leds.

Mine still look new. I'd contact them and see if they'll warranty them.

I did contact form directly and it seems like they are, slow responding. Had to find a bill of sale with product i purchased. Hope it doesn’t happen again. Maybe next ones will have tighter cutoffs!

Well, after about 2.5 years, mine are starting to act up. Realistically, they started acting up a few months ago but couldn't get the issues on video to prove it. Basically the drl would start flickering, but the only time I'd see it would be when driving and when pulled up behind another vehicle, but it will be intermittent. Called them up and tried to get a claim going but since the issue was intermittent and unable to be documented they didn't do anything. That was back at the end of March. Fast forward to Wednesday of this week, I noticed my passenger side headlight had basically reset itself as I pulled into a Napa parking lot, I saw it doing the start up sequence in the window. Called them up again to explain it. They wanted pictures of the form logo on the back of the housings (since there was no serial number on these since they were pre-production). So I pulled the front end apart Friday morning to do some other work and found a bunch of water in the driver side headlight as well AND the projector shroud is rubbing on the outer lens and rubbing the paint off.
So, I took the requested pictures, plus new ones showing the moisture and POOLED water inside the housing and sent the email off. I called back a while later to make sure they got the email. They were waiting for a decision.
They emailed me back a while later saying that they were not going to warranty these headlights because they were outside the warranty period and I didn't have a receipt. Even though Lightwerkz, one of their top sellers and installers did the install and posted it on their Instagram page and could vouch for the sale.
I understand being out of the warranty period, but these were essentially test pieces and you'd think they'd be more lenient on it. On top of that, I have zero doubt that if I had an Instagram or Facebook page of thousands or hundreds of thousands or millions of followers, they would have replaced these things, no questions asked. I was one of the first in the country to have these lights, if not the first outside of their company.

@hunterdan, perhaps you should send them an email with a link to this forum. You've supported their company within your posts and really should have earned the right to replacements.

Being a trailblazer should mean something in my opinion.

I thought I did, checking back through my sent messages, guess I didn't attach the link.

I have been contemplating ordering a set of the Form Headlights, but not if they will only last a few years.

They said they'd offer the latest sale price on another set... Why would I buy another set if they're going to last just long enough for the warranty to fail

Thats a bummer, i still have the replacements they sent me in the box because of the bubbling paint on the 1st set! Maybe i should hold off until these 1st sets take a dump! I dont use dtrl, i figured id save them as i cant replace anything.
Anzo lights are quality, i have their tales with not one issue for 3 years or so. I would wait for black Friday and grab a set on sale.
