That's not much info, but ok...

Sounds like your fan clutch might be bad. It's supposed to engage/disengage based on the temperature of the air coming off your radiator. If its bad it could be stuck in the on position.

Or, the previous owner may have installed a heavy duty fan clutch. HD clutches activate the fan more frequently and at higher speeds than the stock light duty fan clutches. This results in a lot more fan noise from under the hood. Think of HD trucks and 18 wheelers that sounds like an airplane taking off but they're only going 10 MPH.

Do either of these sounds like what you're talking about? Are you hearing the roar of the fan noise?

Thanks guys. But it seems to be coming from the exhaust, as it comes out the passenger side right before the rear tire. This truck has me scratching my head. Changed the oil for the first time yesterday and the oil filter that was on the truck fit but was not what goes on it. Its much smaller. After looking the part number on the filter it would fit older dodge cars. BUT IT FITS. AND WHAT SUPPOSED to go on it wouldn't work as its to big.

I can't say about the 2003 Ram Hemi, but the 2006 Ram Hemi exhaust is a resonator, muffler (looks like a small trash can), and resonator after the catalytic converters. What you are describing sounds the muffler has possibly rotted out.

Does the truck make noise when standing still or only when moving?
Speed related?

Thank you to all who replied. Truck is running much better I have ran a couple tanks of 91 (premium) threw and it made a huge difference. I guess the truck don't like 87

The owner's manual does recommend premium (91 octane), but that has more to do with knock caused by unintended secondary combustion at higher compressions and nothing to do with actual horsepower. You can run with regular (87 octane) and have no real issues to speak of, you may notice your engine shake a little more at idle. Run a few bottles of your favorite fuel system cleaner (mine is Amsoil P.i. Performance Improver Gasoline Additive every oil change and BG 44K fuel system cleaner once a year) through your tank, then switch back to regular and see what your results are. If was carbon fouling (valves and/or injectors) then you should see a noticeable improvement running the 87 octane. Just realize that your engine won't feel as "smooth" at idle running regular gas instead of premium.

EDIT - Another thought that just passed through my brain, you may have a performance tune needing the higher octane. That's just an idea though.
