Well, I finally broke down and got my muffler replaced.
Out with the old trashcan, pipe and resonator (more on that later):

First 'new' piece is a repurposed 3G Ram Cummins tailpipe.
Cleaned it up with a wire wheel and painted it with black high head ceramic paint:

(It's only glossy because the paint hasn't flashed yet here, it's a nice semi-gloss finish now)
So, 18" Center In Offset Out Magnaflow, a 3 - 4" adapter and a small piece of 3" pipe later and we're off n running.

Review: It sounds f*cking amazing at idle, and accelerating. Amazing, f'n love it! I love how the larger pipe (which slipped right into my factory hanger) tucks up nice and close, and ends at an awesome angle to the truck. BUT! Man, cruising around 1,500-1,800 RPMs it has the drone from HELL. I think I understand why there was a big resonator on the truck from the factory.
I am working on a Helmholtz chamber (1/4 wave supressor) to deal with it, but that hasn't gone well so far. More parts ordered, I'll update when that happens.
My other thought is to try n find a factory style resonator and install it upstream to the muffler. Effectively, it shouldn't matter where the resonator is placed.
