Looking to go bigger with 35’s on a 20“ rim. Do i need to add a lift? if not, does it look better with a lift? Suspension or body?

So you have a 2500 yes, with the solid axle right? Do you know the gearing? Yes it will look better with a lift and your truck is built to handle it as opposed to 1500's where you have to worry about every move. If you want a real lift get a suspension lift, some places don't even do body lifts, suspension lift or bust. 35's would probably work, depending how wide you might rub, maybe you will need a cheap level, but it will fill the fender full, I wouldn't like that at all personally. A bare minimum would be 3.92 gears, but you will be much happier with 4.56 with a lifter properly 2500. It just depends on the money you have to invest, it surely wont be cheap to do it right, especially if you do gears.

All you need is a leveling kit. I used the Bilstein level kit with front reservoirs and was an easy install. I then put 295/65R20 (on the factory rims) which are slightly taller than 35's. The ride on my 2500 Mega slightly softened a bit, which was welconed.

I upsized to 35 inch mud tires on my 2500 without a lift or any other modifications. I don't feel any sluggishness, and I didn't really notice any change in my fuel economy. I would, however, get the speedometer recalibrated, because of I set my cruise at 80, I'm actually toodling along at 82-83. Not much, but enough to get pulled over when the speed limit is 75.

I'd find some factory 18's. Tires are cheaper and the ride will be much better with taller sidewalls. Some Method's come in Ram specific backspacing.

I just bought a RAM with 35s on it. It has a 3.55 diff. Power is fine mpgs are ok. BUT the noise of those tires SUCKS. I have thought about selling the rims and tires (FUEL) and going back to stock tread. It has neither a level or a lift on it. It does however have both front liners removed. Which I do NOT like as there is dirt and gravel in there. I will be removing the debris out of it before I start driving it regularly.

Leveled on 35s. 1.5" level. No issues whatsoever.
