Sorry if this has been discussed elsewhere but I have a '17 Ram 1500 Big Horn 5.7 with 230k.
Ran PUP 5W20 til about 135k, developed the Hemi Tick. Switched to RL5W20 and RP filter which helped with the tick and continue to use.
I drive almost all my miles on toll roads/highway, 5 - 10 hours one way and change every 7500.
Have recently developed a rattle noise only on hard acceleration and getting a slight exhaust odor (only noticeable when the sliding rear window is open). I do run 89 octane majority of my fill ups.
I do have a K&N CAI, Magnaflow exhaust and Pedal Commander (which installed after buying the truck new).
Anyone ever experience a similar noise on hard acceleration with exhaust odor and if any opinions on switching to RL 5W30 with my miles would help or not.
Thanks in advance to any comments!!

I believe there is a heat shield by the manifold that sometimes loosen up. That can cause the rattling you describe.

The exhaust clamp near the trans/cross memmber is very common to rot off and make the pipe rattle at least on all of my 4th gens.

230K.........last time plugs were checked?........smell probably unburned fuel in cat & rattle probably is pinging if it only occurs under hard acceleration.
