Start pricing out cam kits (go with a kit from an outfit that specializes in the hemi,like FRP /MMX /Moes etc.) / MDS delete plugs (use OEM plugs) / head gaskets / timing cover gaskets / anti-freeze / oil change ,unlocked PCM and tune.
It's gonna be a few thousand for parts,and if you're paying a shop to do it,add in another thousand or two,then you'll have to have somebody tune it,so add in a bunch more money.
It's not cheap to put a decent quality cam in a hemi, don't even look at cam kits that are only a grand,as they're basically junk

OP, as @Wild one says, it's gonna be expensive. Heck, you could probably buy yourself a new long block for about the same cost.

My recommendation is to do what I did, get an unlocked PCM and tune. You can shut off the MDS completely in the tune. Also, you can adjust everything else in the tune, for more throttle response, slightly more power (if that's what you're looking for), better shifting for the transmission, etc, etc.

Of course, my method does not eliminate the potential trouble-spots (cam & lifters). But if you change oil/filter religiously and avoid long idle times, you stand a decent chance of long engine life.

My ECU is locked and I see the little green light when it goes in that mode. Best I can tell from what I have read is it could be bad on certain lifters? There are folks out there that absolutely hate it. And these damn pop-up ads when I post.

MDS doesn't physically harm the engine, it's just a very noticable effect when it comes on/off in some trucks so I personally prefer to disable it permanently (pulsar).

Some people still think MDS is the reason for lifter failure but that's not the case; both MDS and non MDS lifters fail, and even hemis without MDS still experience lifter failure.

You can safely disable MDS in software, I have about 150k KMs on my engine and short of a few hundred, they were all put on with MDS disabled.

It serves no purpose It saves little to no gas the whole system is emissions based just like stop start. It makes the truck drive like crap especially when on the highway slowing down speeding up kicking on and off ect. Also like myself I have a magnaflow exhaust It would sound horrible if mds was enabled. Some of us also think that keeping MDS makes the lifters fail earlier ect.

Is there a certain year RAM where this became a requirement, available tuners today that require it, or both. Asking, because I turned off MDS with my Diablo 10 years ago on a 2014, I didn't need to buy an unlocked PCM.

2015 and up is locked its just a stellantis thing they dont want us working on our own vehicles.

The MDS system gives me great mileage if I'm doing alot of highway driving.

I didn't notice it much with my stock muffler, other than that hesitation when you accelerate while cruising.

I notice it alot more since I did the muffler delete on my '15 SLT single exhaust. The most annoying is while cruising at 20-30 mph on side streets, I can really hear it lugging if I feather the skinny pedal.

I usually just use my gear - button to disable it in that scenario.

It is an annoyance more than anything else.

Specific trucks seem to suffer from more substantial acceleration hesitations, and some are prone to droning, usually with aftermarket exhaust.

And everyone has a different tolerance for trannies gear searching, lurching or hesitation on slight grade changes.

I use my truck in a similar fashion to Ed. I drive mostly on rural highways and use the ERS buttons on the steering wheel whenever I'm driving in an urban area with traffic.

Automatic gear selection on flat grades isn't too annoying with my truck (for me ).
After extended travel on a slight grade, if the RPM is below 1500, I will tap the ERS buttons to downshift to get the RPM running higher.

I don't appreciate lugging the gears or demanding power at low RPM.

It would be nice if the MDS was only active at 45 mph and above, IMO.
