One of my OEM factory installed pucks does not have metal bar in the guts of the puck. The other three do. I'm curious if this is intentional or if they should all have similar guts. On the one missing, believe it is the rear right - and has a hole for the bar - it's just not there. What does yours have? Thoughts?

I am not sure what you are asking. The sockets in the bed of the truck, are just that sockets. The pucks on the fifth wheel hitch for instance, that are inserted have the locking mechanisms that are turned to "lock" them into those sockets.

5:50 mark on the above video.

The puck doesn't lock in on a bar. It is a rectangular hole that locks in by turning the puck or safety chain loop 1/4 turn. See the video above. (Not my video but you can see the hole it locks into what it takes to install it yourself)
